THE NEED
Up to 25% of hospital admissions could be avoided with early detection and intervention—equivalent to 221 out of 815 admissions, according to the Confederation of Danish Industry. Such preventable admissions strain patients and may worsen their condition, while requiring significant resources from hospitals, municipalities, and emergency services. This highlights a clear clinical need for real-time monitoring and better decision support across sectors.
THE SOLUTION
The project implements digital tools for early monitoring and assessment of patients’ health status within municipal care. This strengthens cross-sector collaboration with hospitals and enables rapid intervention at signs of deterioration. The solution helps reduce unnecessary (re)admissions, improves patient pathways, and ensures more efficient and targeted use of healthcare resources.

PROJECT SUMMARY
